Chart: First and Ongoing Responses

  • Updated

The First Response feature is available to document efficacy and adverse reactions to medications, per the Joint Commission Requirement. The First Response button appears on the first Observed/Administered dose of an order in the Med Log/MAR.

User Permissions: Users who can access medication documentation (Super Admins, Records Admins, Doctors, Nurses, and Techs) all have access to the response areas. Users with Super Admin permissions can set up and edit the relevant settings in Instances or Location Settings.

Med Log

Once responses are set up and a patient receives a dose of medication, the following buttons will be available in the patient's Med Log.

  1. To begin, click on Patients > Patient tile > Med Log.
  2. Locate a medication that has already been administered and has met the minutes until response time specified in Settings > Instance or Settings > Locations.
    • First Response: When the dose administered is the first dose of that medication the patient has received, the response button will say First Response. 2.png
    • Medication Response: When ongoing responses have been enabled, and the dose administered is not the first dose of a medication administered to a patient, the button will say Medication Response. 1.png
  3. When a user clicks on either button, the following pop-up will appear. Both fields are required.mceclip0.png
    • Efficacy: Use this field to document if and how the medication worked.
    • Adverse Reaction: Use this field to document any adverse reactions to the medication. If the patient experienced no adverse reactions, you can simply enter N/A or any similar text.
  4. Click Save Response to continue.
  5. The documented medication in the Med Log will display the responses entered in the Efficacy and Adverse Reaction fields, as well as the name of the user who recorded them, together with the date and time. 3.png

Note: The medication will remain highlighted in red until the Medication Response or First Response field is completed.

Dashboard

Patients appear on Dashboard > Med Pass at the time that the First Response is due (based on the Minute until Response defined in Settings). You will see a red R to indicate that the entry is a Medication Response.
