Quickly add Consent and Evaluation forms to a patient chart using the New Document modal. This functionality centralizes all forms you have access to into one convenient location, allowing you to search for the desired document, create a custom list of favorited forms, and instantly add multiple forms to the patient chart.
Add Documents to the Patient Chart
Let's review how to use the New Document window to quickly add evaluation and consent forms to the patient record.
Required Permissions: Auditors, Pharmacists, and Utilization Review External Users won't have access to the New Document icon. The following roles may have access if combined with another role: Lab Liaison, Lab Report Operator, Lab Requisition Operator, Biller, or Provider. Patient Coordinators will only have access to Pre-Admission Evaluations.
- From the patient chart, click on the New Document icon.
- The New Document window displays all evaluation and consent forms you have access to based on your user function and the patient's location.
- Search Documents: Type in the document or tab name to filter the list of documents to only those matching your search criteria.
- Select Tab: Select the name of a patient chart tab from this drop-down to display only forms in that tab.
- Consent Form: Toggle on to only display Consent forms.
- Evaluation Form: Toggle on to only display Evaluations.
- Note: When both toggles are enabled or disabled, both consents and evaluations will display.
- The Documents column displays an E for Evaluation or C for Consent.
- The Name of the Document is displayed, followed by the Name of the patient process tab.
- Click the Edit icon to add the document to the patient chart tab and automatically open the document for completion.
- Click the Heart icon to save a document to your personal favorites list. Favorite documents appear in the My Favorites section.
- Favorites are specific to you and will be maintained in the modal.
- To remove a favorited document, click on the heart icon again.
- Click the Plus icon to add one or more documents to the patient chart. The New Document window will remain open, allowing you to select additional forms as needed. Each form will be added to the corresponding patient chart tab as listed in the title.
- Click the drop-down arrow to view a history of the added document including the status, creator, and most recent updater.
- The # column displays the number of times that an individual document has been added to the current patient chart.
Disabled/Deleted Documents
When a consent/evaluation template is disabled or deleted, it is removed from the Documents and My Favorites lists in the Add Document modal. When the form is re-enabled or restored, it will return to the add document modal lists, including My Favorites.
User Permissions
Users are only able to see the appropriate forms based on their role, function, access to restricted tabs, and locations.
Within the New Document modal:
- Forms should only be displayed if the current user’s User Function matches a User Function selected in the form template’s Staff Signature User Titles.
- Users should not see any forms assigned to a Restricted Tab that they do not have access to
- Users should only see forms that are assigned to the patient’s location
