Chart: PHI Tab

The PHI tab allows you to log any disclosure of Protected Health Information (PHI).

Logging PHI Disclosure

  1. From the patient chart, open the PHI tab. 
    1. Note: The name of this tab is customizable under Patient Processes
  2. Click Add PHI Entry.mceclip0.png
  3. Complete the Disclosure of Protected Health Information.mceclip2.png
    • Date disclosure: This is the date of the PHI disclosure (defaults to today).
    • Date requested: The date the PHI was requested (defaults to today).
    • Request type: Choose written, verbal, or other.
    • Disclose to: Enter the name of the individual to whom the information was disclosed. You can also include the name of the institution if relevant, e.g., Meg Smith, admissions coordinator at Best Rehab.
    • Address: Enter the individual or facility address to whom the information was disclosed.
    • Description of the disclosure: What was disclosed? 
    • Purpose of the disclosure: Why was it disclosed?
    • Disclosed by: The name of the user who disclosed the information.
    • Attachments: Click Add files, attach any pertinent documentation, such as an ROI, a subpoena, etc.
  4. Click Add PHI Entry to save.mceclip4.png
  5. Entries will be logged in reverse chronological order (newer on top, older at the bottom).
