To report the required data to OBHIS, you will need to make sure that all required data fields are included in your evaluations. These questions are typically included in pre-admission and admission assessments. You can either create new OBHIS-specific templates that include these fields or replace the fields in your existing forms.
Note: Even if you already collect these data points in your existing forms, the field types must be updated in every template to ensure they are connected with the OBHIS state reporting functionality.
Required Field List
The following fields are required to be reported for every Ohio Medicaid patient:

Important: If Mental Health is the primary diagnosis for the patient, the Drug Use History and Medication Assisted Therapy fields are not required.
Consent to Report
Your consent forms should include submitting the patient’s data to the state. You can add this to an existing consent form or create a new consent form specifically for your Ohio Medicaid patients.
Template Modifications
Let’s review how to update Evaluation templates to include the new dynamic state reporting fields. To make modifications to Evaluations, you must be a Super Admin or have the Manage Templates feature added to your user profile.
- Click on the Templates tab and choose Evaluations.
- In this example, we’ll open a Pre-admission Assessment template.
- Notice the new arrow on the right side of the screen. Click on this to display the required fields for OBHIS reporting.
- Here, you can see the field elements. Note that each has a circle. The color of this circle indicates whether this field has been added to forms within your EMR.
- Gray dot: This field is not present in any evaluation.
- Green dot: This field is present in at least 1 evaluation that is enabled
- Yellow dot: This field is included in an evaluation, but the evaluation is disabled.
- Locate the question that you would like to replace. In this example, we are replacing the Current Living Arrangement question.
- First, change the Field Type Source drop-down to dynamic.
- Next, use the Field type drop-down to select the Current Living Arrangement.
- Note that the dot next to the Current Living Arrangement field has changed to green now that the field is used in an active evaluation.
- Repeat this action until every field has been added to at least one evaluation for every location.
Pre-population
Dynamic fields allow for pre-population, just like standard fields. All fields must be set to Dynamic fields with the same Field type selected for pre-population to work.
