Prepare Chart For Labs

Accurate patient information on the facesheet is essential for smooth lab ordering and billing. Completing the key fields helps labs determine who to bill, how to classify the patient, and what requirements apply. Providing this information up front reduces delays, prevents rejections, and ensures the requisition is processed correctly by any connected lab.

Patient Demographics

Each lab must complete the required fields for standard and unique labs before authenticating the chart for placing and receiving orders.

  • First Name: The patient's legal first name. 

    • Note: Adding other names, such as nicknames or preferred names, to this field can cause issues and errors. Please use the Preferred Name field for the patient's nickname or preferred name.  


     

  • Last Name: The patient's legal last name.
  • MR Number
  • Date of Birth
  • Birth Sex

  • Address Line 1
  • City
  • State
  • ZIP

Insurance

Accurate, verified insurance details are key to smooth lab billing. Bad or incomplete insurance info is one of the most common reasons labs delay or reject specimens, or accidentally bill the patient. Always make sure the insurance section is complete and up to date:

Required Fields for All Patients

  • Insurance Payer
  • Subscriber ID
  • Effective Date
  • Full Subscriber details (Name, Birth Sex, Address)
  • Insurance Priority
  • Plan Type
  • Payer Type
  • Insurance Phone Number
  • Payer Address

We recommend upload a clear image of the insurance card and use VOBGetter to confirm eligibility before ordering labs.

Lab Testing

To ensure the laboratory receives accurate billing information and can process payment correctly, complete the lab billing fields on the patient's facesheet for any patient who will have lab testing. 2025-12-01_13-19-04.png

  •  
    • Lab Bill To: Select Patient, Facility, or Insurance from the drop-down list to indicate who is responsible for the lab bill.
      • If the Lab Bill To field is set to Insurance, select Primary Insurance from the Lab Primary Insurance drop-down. Insurance will only display in the drop-down if all fields have been completed in the insurance section of the patient's facesheet. Only active insurances will appear in this drop-down. 
    • Lab Guarantor Type: Select Self, Spouse, or Other from the drop-down to indicate who is the lab guarantor.
      • If the Lab Guarantor Type field is set to Self, the patient will automatically be added in the Lab Guarantor field.
      • If the Lab Guarantor Type is set to Spouse or Other, select the correct patient contact in insurance subscriber from the Lab Guarantor drop-down. 

        • Contacts will only appear in the drop-down if the following fields are filled out:

          • Phone
          • Address Line 1
          • City

    • Lab Patient Class
    • Lab Guardian (if patient has a legal guardian)
