Bill by Building

Bill by Building allows you to bill patients in one EMR Location to different CMD Facilities or RCM Practices by using their Building and Bed assignments. This supports scenarios where you need to submit claims with different billing addresses, Tax IDs, NPIs, or separate credentials from the same EMR Location.

Depending on your integration, those billing entities can be:

  • CMD Facilities (for CMD billing), or
  • RCM Facilities (for RCM billing).

Important: Bill by Program and Bill by Building should not be enabled in the same EMR Location at the same time. You should only have one of these options enabled and in use. 

Create Buildings

Ensure Buildings are created before you begin by following these instructions.

Once a Building is created, any patient assigned to a Location must have both the proper Location and corresponding Building added to their facesheet.

Enable Bill by Building

The Billing by Building setting must be enabled by Kipu staff. If it is not already enabled for your instance, please contact Billing Support to get started.

Once enabled for your instance, you’ll turn it on for each Location.

  1. To begin, click on your initials and then click Settings
  2. Then click the Billing Audit Tool tab.
  1. If BBB is enabled, make sure that the feature is also enabled under Settings > Billing Audit Tool > Location Settings:

  2. Next, click Location Settings.

  3. To update a Location's settings, click the Edit icon. Edit Location Settings
  4. Select Enable Billing by Building and click Submit.

  5. Repeat for additional Locations as needed.

Konnector Setup by Integration Type

CMD (CMD Facilities)

For CMD billing, each CMD Facility must be configured to a Konnector in the EMR to create the integration between the platforms.

To request a new CMD Konnector, please complete the CMD Add Konnector Request form. If you need to modify an existing CMD Konnector (for example, to point to a different CMD Facility or update credentials), click here. 

RCM (RCM Practices/Facilities)

For RCM billing, a Konnector in the EMR must be configured for each RCM Facility that needs to be integrated with the EMR. Click here for instructions on configuring RCM Konnectors.

Note: If you need additional Konnectors created to support Bill by Building, please contact Support.

Example Configuration

Let's review an example:

I need to integrate my two EMR Locations, Treatment Center and Outpatient Center, with my three RCM Facilities. I also want to manage my Treatment Center billing based on Building.

My Treatment Center contains three Buildings:

  • Red – Detox and PHP patients
  • Blue – Male in-patient population
  • Green – Female in-patient population

The Outpatient Center doesn't have Buildings, so I don't need to configure Bill by Building for that Location.

I want to bill:

  • The Outpatient Center separately
  • The Red Building separately
  • The Blue and Green Buildings together

Tips for Using Bill by Building

Let's review some best practices for using Bill by Building:

  1. The patient must always be assigned a Building and a Bed.
  2. The system will use the Konnector that matches the Location/Building/Bed assigned to the patient on the billable item's Date of Service.
  3. When two Buildings are assigned with the same start date, the system will use the most recently assigned Building for the billable items.
  4. When making retroactive changes to the patient chart (like changing the Building/Bed assignment), please wait 5 minutes before making another update to the same object.
  5. Always make sure to delete any conflicting Building assignments with the same start date. Conflicting building assignments

If you are making retroactive changes to a patient whose documented services have already been generated on the billing report with the incorrect information, please contact Support for assistance in updating the services to use the correct data.
