Patients receive a discharge plan tailored to their specific needs, which clearly outlines various services and support resources, such as referrals to outside providers, medication-assisted treatment, and housing assistance. The care team collaborates with the patient to create the discharge plan, encouraging the patient to actively participate in their aftercare.
Important: This process only describes the discharge process for clients not using a CRM or integrated RCM platform.
Here are the best practices workflows to conclude an episode of care:
- Discharge Workflow applies when the patient completes their episode of care or completes treatment, transitions to an outside facility or provider, voluntarily withdraws from treatment, or when the facility decides to end treatment due to financial, behavioral, or operational reasons.

Transfer Workflow applies when a patient moves to another substance use treatment program within the same Kipu instance, such as a location offering a higher or lower level of care or a location that specializes in a particular treatment type.
