A requisition is a form that will need to be processed at the time of the specimen's collection. The requisition will display the patient's demographics and all information that was documented on the order. Once processed, this requisition will be sent electronically to the lab.
When the specimen is received by the lab, they will use the requisition's ordered test to run the specimen. As the requisition will pull all of its information from the order, no requisitions will be available if an order has not been placed on the patient.
Required Permissions: To create a new requisition, you must have Lab Requisition Operator role or be Super Admin or Records Admin.
Overview
To manage requisitions, click Labs at the top of the screen, and then click Lab Test Requisitions.
The Lab Test Requisitions tab allows the user who will process the requisition to control sent or created requisitions. If a requisition cannot be sent due to a requirement, it will be listed here as well.
Processing a New Requisition
Let's review how to create a new Lab Requisition.
Note: Lab Requisitions are only available for patients who are admitted and have an active Lab Order. Expired orders will not be available and the facility must enter a new order if testing is required.
- From the Lab Test Requisitions tab, click Create.

A list of patient requisitions available to process today will display (as determined by the order's frequency).
- The user can also see requisitions that are due tomorrow, on a custom date (click on Forecast and select a date from the pop-up calendar), or see all patients that have a requisition available for processing by clicking Manual.
- Some labs require that requisitions contain orders from only one provider. When this requirement is enabled, a patient's active requisitions will be grouped by provider, and each requisition will have a unique label.
- Click the Create button to build the requisition.

Specimen ID and Labels: At the top of the requisition, the user will see the Specimen box. This is where the user will scan the barcode on the labels provided by Kipu or the lab, OR generate a unique specimen ID, and/or print it using a Dymo printer.
- If you're using DYMO printer, click on the label button to generate the Specimen ID and print the label. The label will automatically include the patient's name, DOB, MR#, collection date and time, and patient's location.
- Click Print Label and attach it to your specimen container.
- If you're using a label from pre-printed roll of labels, pick the label and fill it out with the patient's information, such as Name, Collection Date, DOB, and signature, as determined by your facility's or lab's requirements.
- Next, click the Specimen ID box and scan the barcode. This will populate the code into the box. If the scanner is unavailable, the user may type the code into the box. This code is case-sensitive, so enter it exactly as it is printed on the label or you'll receive an error.
- Please note that the Specimen ID must be unique, so once a barcode ID is used, it cannot be used again.
- If you run out of specimen labels, please contact us at labteam@kipusystems.com, and we will provide you with a batch of temporary barcodes to print, cut, and paste onto your specimen containers for your immediate needs. We will also ship additional labels to you.
- Next, click the Specimen ID box and scan the barcode. This will populate the code into the box. If the scanner is unavailable, the user may type the code into the box. This code is case-sensitive, so enter it exactly as it is printed on the label or you'll receive an error.
- For LabCorp requisitions, the specimen ID will generate automatically, and you do not need to print the label at this stage.
- If you're using DYMO printer, click on the label button to generate the Specimen ID and print the label. The label will automatically include the patient's name, DOB, MR#, collection date and time, and patient's location.

The Collected At date will default to the current date and time.
- If the specimen was collected on a different date and time, you can change it by clicking the box and making the changes on the pop-up calendar. When the collection date and time have been set on the calendar, click Done.
- If the specimen will be collected at the lab, select Off-site collection.
- If the specimen was collected on a different date and time, you can change it by clicking the box and making the changes on the pop-up calendar. When the collection date and time have been set on the calendar, click Done.

Medical Test Orders: This section contains the tests/panels they are ordering with this specimen. This information is pulled from the Lab Orders entered on the patient's chart by the physician or nurse.
- You may deselect tests you do not wish to run for this specimen during this step.

Point of Collection Results (POC): Some lab interfaces will offer the ability to document point of collection results. If they are available on the requisition, the user can document the results accordingly. These results will be saved to the requisition and can be pulled later by the facility if needed. If a POC question is not answered, the test won't be documented.
- Some labs require additional details before requisition can be sent.

Clinical Data: Review the patient's current diagnosis and any prescribed medications at the time this requisition was created.
- Sign Consent for Testing: Before the requisition can be sent to the lab, the patient must consent to the testing. Click Sign Consent for Testing.
- Click Save or Save & Send to send it to the lab.
- If the patient is not available to sign the consent, click Save so that the requisition can be edited once the patient is available. The requisition won't be sent until Save & Send is selected.
- If you encounter an error (e.g. missing Dx Code, invalid patient, etc.), your requisition may not be sent. Correct the error and click Save and Send again. If that doesn’t work, you need to start a new requisition. Make sure to copy the specimen ID or reprint the label.

LabCorp requires a special printed requisition to be included with each specimen. Once printed, peel the label from the bottom of the requisition and place it on the collection container. If you run out of the requisition paper, contact your LabCorp representative.
Note: If your requisition contains more than 40 tests, or orders from multiple providers, or special types of orders (e.g. cytology, microbiology, etc.), the system will automatically split it into multiple requisitions. You will need to print all of them together and include them with your collected sample.

Quest Diagnostics requires that a printed requisition be included with each specimen. Once you click Save and Send for the requisition, the system will offer you the option to print the requisition immediately.
If the patient is going to be tested at the Quest location, give the printed requisition to the patient.
Note: If your requisition contains more than 35 tests, the system will automatically split it into multiple requisitions. You will need to print all of them together and include them with your collected sample.
Managing Lab Requisitions
All requisitions that were sent, created, or are on hold due to a requirement, will display on the list in the Lab Test Requisitions tab.
Lab Requisition Statuses
The Lab Test Requisitions tab lists all patient requisitions with color-coded status labels.
- Created: Requisition was created and saved, but not sent to the lab.
- Sent: Requisition has been saved and sent to the lab
- No Consent: Requisition was created and saved, but still requires the patient's signature before it can be processed. The requisition has not been sent to the lab.
- No Form: Requisition was created and saved, but still requires a signed consent form provided by the lab in order to be processed. The requisition has not been sent to the lab.
- No Review: Requisition was created and saved, but still requires the ordering physician to review (sign) the order for testing to be processed. Click here for instructions. The requisition has not been sent to the lab.
Important: Requisitions that are highlighted in blue indicate that their results have been released to the EMR.
Lab Requisition Actions
In the Lab Test Requisitions list view, users may take additional actions with the requisitions. Available actions include:
- Edit Requisition: For cases when requisition was created but not completed, including acquiring a Patient Consent for Testing (only available on requisitions that have not been sent).
- Delete Requisition: For cases when the requisition was created in error (only available on requisitions that have not been sent).
- Send Requisition: When the requisition is complete, and all other requirements are met (only available on requisitions that have not been sent and have no holds).
