Rounds: Manage Rounds

This article describes how to log patient activities with Rounds. We'll cover the following workflows: 

  • How to start a round
  • How to navigate the interval page
  • How to document patient observations
  • Round and interval statuses

Please Note: We recommend completing the exercise in Fullscreen by clicking on the icon on the top right corner.

Start a Round

Different users will see different rounds available for them based on the user's permission and round's assigned leader(s). All rounds available for the users to start at this moment have a blue background. 

  1. To start a round, navigate to the Shifts (or Rounds) tab on the top ribbon. mceclip0.png
  2. Open the Rounds tab.
  3. Click on the title of the round you need to start.mceclip1.png 
    • Note: You can only start the rounds within the round's start and end time range. If the round was not started prior to its end time, there is no way to start the round or log observations for that round.
  4. Once the round is started, the round interval page automatically opens and the status changes from Pending to In Progress.mceclip2.png

Interval Page Details

Starting the round automatically opens the current interval page. The interval page displays information about the round and the current round's interval and allows you to document patient activity and location for the round interval.

  1. Round Details: This section contains the title of the round, interval duration, the round date, and the round start and end times. 
  2. Round Leaders: Displays the user(s) assigned to this round as its leader. This can be blank if the round template assigns leaders based on role or function.
  3. Building: Displays the building this round is tied to (if applicable). 
  4. Add Round Leader: This button allows users designated as the current Round Leader, Super Admins, or Records Admins to assign additional round leaders as needed.
    1. Click on the Add Round Leader button on any interval page for the round.
    2. Use the drop-down to select a user. Only users eligible to manage this round based on the template setup will show up on the list.
    3. Click Add Selected. You can add as many Rounds leaders as you need, however, you cannot remove previously assigned Round leaders.

  5. Current Interval: This displays the total number of intervals for the day and the current interval selected. You can use the Select interval drop-down to navigate between the current and past intervals for the current day. 
    • You will also see a countdown to the next interval on the current interval (if the interval isn't the last of the round).
  6. Unobserved Patients: This section contains a list of patients that need to be observed before the interval can be completed. 
  7. Completed Patients: This section contains a list of all patients that have already been observed. 

Document A Round Interval

To successfully document a round, each patient within the interval must be observed.

  1. Use the Patient Search option to quickly locate the patient by first or last name. You can also sort the list by Occupancy or Name.mceclip4.png
  2. From the Interval page, select the Activity and Location from the available drop-downs and click the Observe button. The Observe button cannot be clicked until you select an Activity and Location as both values are required for the observation. mceclip2.png
  3. To log additional details like Vitals, Orthostatic Vitals, Glucose, Height/Weight, or add Notes, click on the Assessments button.mceclip5.png
  4. To observe the patient from this viewin addition to Activity and Locationenter valid data for the included assessments or check No entry. You can adjust the Date/Time for the assessments as needed. mceclip6.png
  5. Click on the Observe button when done.
  6. You can only observe each patient once during the interval. 
  7. The interval will be updated as Complete when all patients have been observed. mceclip7.png
  8. Once all intervals are Complete and no additional intervals are scheduled for the day, the Round status will also be updated to Complete.mceclip8.png
  9. At that time, a Rounds Signature can be collected to update the status from Complete to Reviewed. 

Document Missed Intervals

Once the round is started, you can document any previously missed round intervals by switching the interval in the drop-down and adding observation logs for the patients.

Delete Observation Details

Super Admins and Records Admins can delete observation details added by mistake for the patient within the interval's observation log. After that, the designated users can re-log their observation notes.

Rounds Intervals and Statuses

Rounds have the following statuses:

  • Pending: The round has not been started yet.

  • In Progress: The round is in progress, not all intervals have been logged yet, or there are missed intervals.

  • Complete: All intervals have been completed for all patients for this round.

  • Reviewed: The round has been reviewed and signed by a designated user.

Each interval within the round may have one of the following statuses:

  • Incomplete: Some patients have not been observed yet.
  • Complete: All assigned patients have been observed.

Each patient has individual statuses for Last Observed time and Missed intervals:

  • Time of last observed interval

  • Not Yet: No observations have been logged for the patient yet

  • # of Missed Intervals: This is color-coded based on how many intervals were missed

The countdown icon is also color-coded:

  • Green: The new interval has just started and there is enough time to document the observations.

  • Yellow: The next interval is in less than 1 minute.

  • Red: There are less than 30 seconds left until the next interval.
