How Do I Disable a Lab Form That Won’t Let Me Edit It?

Issue

We changed the lab that we are working with and I can't edit the old lab form to stop it from appearing on the patient chart.

Environment

Kipu EMR, Roles: Super Admin, Features: Manage Templates, Templates: Consent Forms

Resolution

Lab Consent forms are created and locked by the Lab team. To disable or edit the form please reach out to the lab team with the request.

Please provide your facilities URL and the name of the Consent form within the email.
