Release of confidential information forms can be automatically generated for each of the contacts already present on the patient’s facesheet when the form is added to the patient chart. This workflow will only work for contacts already present on the factsheet.
Follow the steps below to create the release of information documentation for all the patient’s contacts.

To begin, click on the Edit Patient button in the Information tab.
- Locate the Contacts section of the facesheet and click Add Patient Contact.

The Full Name, Relationship, and Phone fields are all required.
- Fill out the remaining fields as needed. Click Add Patient Contact to add additional contacts.
- Click Update at the bottom to save your changes.
- To obtain the Release of Information form, click on the Releases tab, or on the tab designated by your facility, and select Add form.
- Select the Release of Confidential Information with the rule all_patient_contacts.

This form will now be added to the tab.
- Open the Release of Confidential Information form and collect the necessary signatures.
- Note: The patient can revoke the Release of Confidential Information form at any time. Click here for more information on adding and revoking consent forms.
