Release of Confidential Information Without Rule

Release of Confidential Information forms can be automatically generated and the name of the facility can be manually added.

Follow the steps below to create the Release of Information (ROI) documentation for the desired facility.

  1. To begin, click on the Releases tab, or on the tab designated by your facility, and select Add Form.
  2. Select Release of Confidential Information.
  3. The form will be added to the tab with an empty title.
  4. Open the Release of Confidential Information form and type in the correct information in the Name of Person/Agency field and add any additional contact information. Then, collect the necessary signatures. 
  5. The system displays the name of the agency in the form title with the expiration date.

Note: The patient can revoke the Release of Confidential Information form at any time. Click here for more information on adding and revoking consent forms.
