Annotations

Annotations function as addendums to completed documentation, allowing you to add additional pertinent information. You can add annotations to the following patient chart items:

  • Consents
  • Evaluations
  • Flags
  • Orders (Action or Medication)
  • Lab Orders
  • Lab Requisitions
  • Lab Results
  • Group Sessions ( Group Sessions Tab in the patient chart)

Adding Annotations

You can add as many annotations as needed. These notes can be viewed by any user who has access to the document and revised by Chart Auditors or Super Admins. Additionally, any user who can annotate can also revise or expand the annotation, but only Super Admins can remove or delete them.

Note: Evaluation forms can only be annotated in print/preview mode.

  1. Open the item that you need to add additional information to. 
  2. At the end of the document, click Annotate. mceclip0.png
  3. Write your note. Click Create. mceclip1.png
  4. Users can edit an existing annotation by clicking Revise. mceclip2.png
  5. To save changes, click Create. mceclip3.png
  6. The revised note will text will display a strike-through, with the new note below, along with the time and user stamps of the revision.mceclip4.png
  7. To add additional information, click Expandmceclip5.png
  8. Type in the desired addition, then click Create. mceclip6.png
  9. Expanded notes appear below the original, in a thread, along with the time and user stamps of the person who added the additional information. mceclip7.png
