Chart✓: Using Chart✓

Chart✓ allows users to set deadlines and track the completion of patient forms to ensure the facility remains compliant at all times. Use Chart✓ to create a to-do list of all documents that need to be completed for each patient.

This article focuses on using Chart✓ to remain in compliance on the patient chart and dashboard. For information on setting up Chart✓ click here

General Chart✓ Information

Regardless of location, the colors of the Chart✓ items allow users to identify status at a glance.mceclip23.png

  • White: Form is not due for 25+ hours, the event prompting the due date has not occurred yet, or the form is completed.
  • Yellow: The form is due within the next 24 hours.
  • Red: The due date has passed and the form is overdue.

View Chart✓ on the Patient Chart

The Chart✓ tab on the patient chart will automatically populate with the forms and due date parameters as added to the Chart✓ template for the patient's assigned program.

If the patient doesn't have an assigned program and/or location that matches an active Chart✓ template, nothing will display. 

To access the Chart✓ list:

  1. Open the patient chart and click Chart Summary >Chart✓.mceclip16.png
  2. Use the available filters to narrow down the form list, as needed.
  3. From the Chart✓ list you can view the following information:
    • Name: The name of the form which may include the evaluation date, evaluation name, consent title, treatment plans, etc. 
    • Program: The name of the program in which the patient is enrolled.
    • Created by: The user who added the form to the patient's chart
    • Last Updated by: The user who last edited the form
    • Status: The form's current status. Available options include:
      • Not created: The form has not been added to the patient's chart
      • Open: The form has been added to the patient's chart, but has not been started
      • In progress: The form has been opened
      • Ready for review: The form is completed and awaiting review from an appropriate user
      • Complete: The form has been completed and all necessary signatures have been collected.
    • Assignee: The list of functions assigned to the form, including patient and guardian/guarantor tags
    • Due: Due date. This will also list any conditional form due date rules (e.g. 6 hours after UR LOC changed)
    • Recurrence: Any form that is marked to reoccur at specific intervals will display this symbol: mceclip21.png
  4. To complete an outstanding form, click on the form name. mceclip22.png
  5. Complete the form and gather any necessary signatures. The list will automatically update once the form is complete.
  6. If a chart's status is not created, then you can add it from the patient chart by clicking on the title. 
    1. The appropriate tab will open. Then click the Add Form button. 
    2. Locate and select the correct form.
    3. Once the form is added to the patient chart, the status will change to open.

View Chart✓ in the Dashboard

From the Dashboard, staff members can track all pending required documentation for patients across locations and programs from the Chart✓ tab.

Use the All Chart✓ Items tab to work all items in the location. Alternatively, use the My Chart✓ Items tab to review a list of the forms filtered for your specific user function.

  • You can easily filter the Chart✓ Items list using form status, care team, program, or due date.
  • As when working with Chart✓ on the patient chart, click on the form name to open the form (if already added) or the tab where the form should be added to complete upcoming and overdue items. mceclip28.png
    • The columns are as follows:
      • Patient: The patient's name as indicated in the facesheet
      • MR #: The MR number assigned to the patient
      • Admission Date: The patient's admission date
      • Discharge Date: The patient's discharge date
      • Name: The name of the form which may include the evaluation date, evaluation name, consent title, treatment plans, etc. 
      • Status: The form's current status. Available options include:
        • Not created: The form has not been added to the patient's chart
        • Open: The form has been added to the patient's chart, but has not been started
        • In progress: The form has been opened
        • Ready for review: The form is completed and awaiting review from an appropriate user
        • Complete: The form has been completed and all necessary signatures have been collected.
      • Assignee: The list of functions assigned to the form, including patient and guardian/guarantor tags
      • Due: Due date. This will also list any conditional form due date rules (e.g. 6 hours after UR LOC changed)

  • Alternatively, you can click on the patient's name in the Patient column to navigate to their Chart✓ tab, where you can work the forms as established in the section above.
