Assign a Care Team for each patient, designating a Primary Therapist, Case Manager, Primary Physician, Primary Nurse, and any other custom Care Team member your organization uses.

Set up Care Team
From the Patient Settings tab, locate the Care Team functions section. Tip: Use Ctrl + F (Windows) or Cmd + F (Mac) and enter "care team" to quickly locate the section.
- By default, each care team contains a Case Manager, Primary Nurse, Primary Physician, and Primary Therapist.
- You can add additional roles to the care team by clicking Add Item. Name the role by replacing any spaces in the role name with an underscore. Click Update to save.
- You can disable any of these roles by clicking on the Disable icon. Disabled roles have a green checkmark.
Set up the Assignment of Care Team in an Evaluation
Care teams are assigned to patients using an Evaluation template. The system will automatically generate Evaluation Field names for each care team member added to your Patient settings.
- Click on Templates and choose Evaluations.
- Create a new evaluation or edit an existing evaluation.
- To add a new field, click Add item under the Evaluation Items section.
- Set up the Care Team field as desired.
- Select the appropriate care team member from the Field type drop-down. Care team naming convention includes:
- care_team.Case_Manager
- care_team.Primary_Nurse
- care_team.Primary_Physician
- care_team.Primary_Therapist
- care_team.custom_function1 (in the screenshot above: care_team.Secondary_Therapist)
- care_team.custom_function2 (in the screenshot above: care_team.UR_Manager)
- Repeat this process to add all desired Care Team members to the evaluation.
- Click Update to save.
Assigning the Care Team
Let's review how to assign a care team to the patient.
- From the Patient Chart, open the tab where the Care Team Assignment Evaluation is located.
- Click Add Form.
- Select the form from the list.
- Click Edit to complete the form.
- Use the drop-downs to assign each care team member.
Please note that your organization may choose to assign the care team in more than one evaluation, may assign partial care teams, or provide an initial care team and update as needed.
Viewing the Care Team
The Care Team will display on the top of the Patient's Information tab (facesheet).
Additionally, clicking the Care Team link in the header will display the patient's care team when the user is in any other chart tab. This is useful when auditing a patient chart, allowing QA users to easily determine the responsible party.
