Closing/Reopening a Patient Chart

After a patient is discharged and all documentation is finalized, you can close a patient chart.

  1. To access discharged patients, click the Chart Audit filter on the Patients tab.mceclip0.png
  2. Open the patient chart.mceclip1.png
  3. Click Close Chart.mceclip2.png
  4. The system will automatically flag any incomplete items that need to be addressed. These items will be listed under Errors found. Each item must be completed or removed before the chart can be closed.mceclip4.png
    • Once you've updated any incomplete items, click Close Chart again.  
  5. If all the chart's content is complete, the chart will be closed, and a confirmation message will be displayed on the top of the Patient tab. mceclip5.png

Search for Closed Charts

When searching for a closed chart, the header will display (Closed Chart) next to the patient's MR Number.

Reopening a Patient Chart

If needed, a patient chart may be reopened by a Super Admin or Records Admin. This can be useful if something needs to be added to complete the patient's episode of care.

  1. To reopen a chart, use the search bar in the header to locate the patient. mceclip7.png
