Chart Tracker allows you to readmit and transfer patients across locations. The Chart Tracker tab allows you to view a history of transfers/readmits and create Chart Tracker packages to control which consents and evaluations transfer with the patient.
Required: Users must be Super Admins, Records Admins, or have the Manage Chart Transfers feature added to their user profile to use this tab.
Chart Tracker Tab Overview
The Chart Tracker tab has two sections:
- History: This tab contains a read-only list of all transfers and readmits within your instance.
- Manage Packages: This tab allows you to create packages of consents and evaluations that should be included in the readmit or transfer chart. You can create as many packages as needed.
To access the Chart Tracker tab, click on your initials and choose Chart Tracker.
History Tab
The History tab there contains a read-only list of all transfers and readmits.

Click on the + icon to view which forms were transferred between the charts.

Click on the Source Document link to open the original document.
- Crossed-out items indicate the forms (or the charts) were deleted from the system.

Transferred or readmitted charts stuck in the Pending status can be canceled or resent using the Resend or Cancel icons.
- Note: When the retry is successful, the user must resend the transfer/readmit package with documents (if any) one more time using Live Forms Transfer feature.
Manage Packages
Create Chart Tracker packages to ensure the correct consent forms and evaluations are included when readmitting or transferring the patient. Click here for instructions.
You can make updates to packages using the Edit icon on the Manage Packages tab.
