Issue
Staff members should be able to see an evaluation form, but can't.
Environment
Kipu EMR, Evaluations, Consents, User Functions, Users, Role: Super Admin, or both Features: Manage templates and Manage users
Resolution
There are two separate areas within the Kipu EMR that need to be set up correctly for staff to use Evaluation forms in the tabs.
First, confirm that the Evaluation template is set up correctly.
- Select the Templates tab and then the Evaluations sub-tab.
- Locate the form in question and then select the edit pencil.
- If you have a large number of forms, you can search for them by typing ctrl+f on a PC or command+f on a Mac, and typing the name of the form.

- Verify that the user title/function needed is checked in the Staff signature user titles box. If it isn't, please check the related function box and update the form.
If the template is set up correctly then go to the user profile of the staff member who is not able to use the form.
- Select your initials in the top right corner of the Kipu EMR, then the Manage Users option in the drop-down.
- Locate the user profile by either entering their name in the search bar or by selecting the first letter of their last name. Once located, select the Edit button.
- Scroll to the bottom of the User Profile page and locate the Function drop-down. If it is blank then they will not have access to any forms at all. If they have one assigned but it is not an option checked in the template, that is why they're not seeing the specific form in question.
- If you add or change the function, be sure to update the profile.
The user can now refresh their screen or log back in and access the form.
