Kipu allows you to add a subsequent evaluation to the patient chart using the field type option create_evaluation. The claim format and code of the subsequent evaluation will be automatically added to the billing report upon creation. This functionality only applies to billable, ancillary evaluations with a single code added to the template.

First Evaluation
To begin, create an enabled evaluation template.
- You will specifically want to create a billable evaluation. It can include multiple kinds of codes, but only one of each code.
- In the Evaluation Item section, choose create_evaluation from the Field type drop-down and make sure the item is enabled. Add other information as needed.
- Click Update at the bottom of the evaluation.
Subsequent Evaluation
Once you've created the original evaluation, create and enable the secondary evaluation.
- Create the secondary evaluation, marking it as both billable and ancillary, and including the desired code from the first evaluation.
- In the Evaluation Item section, create the evaluation, as desired. Be sure to enable the item.
- Click Update on the secondary evaluation.
- Note the evaluation ID.
- Return to the first evaluation and add the evaluation ID to the Matrix column names, limit with |, Evaluation ID field.
- Click Update.
Patient Chart
Next, you'll need to add the first evaluation to the patient's chart.
- Navigate to the necessary tab in the desired patient's chart and add the first form you created.
- Fill out the form and be sure to check off Yes under Create Evaluation. Then click Update.
- The second form will automatically appear in whichever tab was indicated in the Patient Process field (in this example, the first and subsequent evaluations had the same tab selected). Fill out the subsequent evaluation as normal.
- The subsequent evaluation will automatically appear under the patient's billable items if the template requires no signatures. Otherwise, it will appear as soon as the evaluation is started.
