ChartTracker allows you to transfer or readmit patients with their live documentation and send forms from one episode of care to the next. Transfers can include completed Consent Forms and/or live copies of selected Evaluations. Live copies of Evaluations will appear in the destination chart as fully editable and unsigned; Consent Forms will transfer as completed with signatures intact.
With ChartTracker, you can easily:
- Readmit patients to the same location, with or without a readmit package (consents and evaluations).
- Transfer a patient chart from one location to another, with or without consents/evaluations, within a master instance.
- Send live forms from the previous episode of care to the next episode of care for readmitted or transferred patients.
- View Readmit/Transfer history for your organization.
Important Note for CRM Users
If your organization is integrated with the CRM, any new patient records pushed from the CRM and accepted into the EMR will not allow live form transfer from any previous records the patient may have had in the EMR. Only charts readmitted/transferred through Chart Tracker have the ability to transfer forms. When the chart is transferred and readmitted from the CRM, this bridge for live forms is not established.
Instance Settings
Super Admins can complete the following Instance setup.
- Click on your initials and select Settings.
- Open the Instance tab.
- Check Chart Tracker in the Patient Options section. Tip: Use Ctrl + F (Windows) or Cmd + F (Mac) to search for Chart Tracker.
- Scroll to the end of the page and click Update to save changes.
Important: Enabling this feature replaces the existing Readmit functionality. The feature does not replace the existing chart transfer functionality, you can use both transfer options as needed.
User Permissions
Users with Super Admin and Records Admin roles have access to all Chart Tracker features by default. Users with Admin, Patient Coordinator, or UR Internal roles can readmit patients and view Chart Tracker History.
In addition, Super Admin or user with Manage Users feature can grant the users the following features:
- Manage Chart Transfers - allows the user to use Chart Transfer feature.

Manage Transfer Packages - allows the user to create readmit/transfer packages.
Create Chart Tracker Packages
Super Admins, Records Admins, and users with the Manage Transfer Packages feature can create Chart Tracker packages. Chart Tracker Packages allow you to choose which consent forms and evaluations are included when readmitting or transferring the patient.

To access the Chart Tracker tab, click on your initials and choose Chart Tracker.
To create a new Chart Tracker package, open the Manage Packages tab. -
Click New Package.

Give your package a Name and check the Enabled box. If the enabled box isn’t checked, the package will not be available for selection from the patient chart.

Next, use the search bars to add Consents and/or Evaluations to the package. Note: Users with master instances must ensure the selected forms exist in both the from and to transfer locations. If a form doesn’t exist in the transfer location, it won’t be copied into the new chart.

Check the box next to each for you’d like to add.
- Important: All consents are transferred with the data and signatures and have a Complete status, while Evaluations transfer the evaluation content and have an Open status. Evaluations must be reviewed and re-signed before they can be completed.
- Note: Only the most current version of the form added to the patient chart will be transferred (e.g., if you have three completed Progress Notes, only the most recently completed Progress Note will be transferred, not all three).
- When complete, click Save.

Make updates to packages using the Edit icon on the Manage Packages tab.
