The Treatment Plan template is a special type of Evaluation form that is designed for logging a patient's treatment plan, status, and progress within the facility.
The treatment plan differs from standard evaluation forms in the following ways:
- Once the form has obtained the required signatures (at least one patient, staff, and review signature is required), the treatment plan stays available for editing showing an In Use status on the patient's chart instead of moving to the traditional Completed status all other forms show once signed.
- While the form is In Use, the users with the Therapist role or Edit Treatment Plan feature are able to open the form and add Statuses for objectives and interventions (Target date, Status, Date/Comment).
- Note: A Super Admin can customize these statuses under Settings > Golden Thread > General > Treatment Plan Statuses.
- To close the treatment plan, a user with the role of Therapist or the Edit Treatment Plans feature must click the Close Treatment Plan button to change the form from In Use to Completed.
- Additionally, a user with the Chart Auditor role can mark the Treatment Plan as completed.

Users with the Therapist role or the Edit Treatment Plans feature can add goals, objectives, interventions, and statuses to treatment plan evaluations in the edit view even after they're signed and no longer in the In Use status. Other users will only be able to see the read-only view.

- Only one problem is allowed for each treatment plan.
There are two types of treatment plan forms:
- Golden Thread treatment plan. Read more details on how to build a Golden Thread Treatment plan in the Golden Thread: Evaluation Template article.
- Regular treatment plan. This is discussed further below.
Build a Treatment Plan Evaluation
Super Admins or users with Manage Template or Manage Templates - Consents and Evaluations can configure Treatment Plans under Templates > Evaluations.
Use the following settings to create a Treatment Plan:

Evaluation Content: Change the value to Treatment Plan. That will ultimately identify the form as a treatment plan and allows it to be put in the In Use status instead of the Completed status once signed.

Master treatment plan category: Specifying the modality for the treatment plan. This optional field is used to populate a default modality in Golden Thread treatment plans or list a treatment plan in the correct section of the Master Treatment Plan.
- Note: Super Admin or a user with the Manage Golden Thread feature can set up the modalities under Settings > Golden Thread > General > Treatment Modalities.

Sign in any order for External Signatures allows the guardian or guarantor can sign after the staff member. Click here to learn more.
Note: At this time, signing with PIN does not work for treatment plans with Sign in any order functionality.

Review Signature Satisfied: Allows users who are selected for both Staff and Review signatures to sign once instead of twice. Click here to learn more.
- Note: This setting cannot be used when Force Signatures is enabled.
Important: To use the Sign in any order option, the template requires signature selections for the Patient, Guardian and/or Guarantor, Staff, and Review Signature.
Evaluation Fields
When building a treatment plan template, you can use any evaluation field as needed. There are special field types used for the treatment plan forms, some are designated for standard treatment plans and others are specifically for Golden Thread forms. We do not recommend combining fields designated for different treatment plan types in one form as it can cause the form to not load when added to a patient chart.

Standard Treatment Plan
For a regular treatment plan, you will need to manually build problem(s), goals, objectives, and interventions with frequencies.
Treatment Plan Column Titles
The treatment_plan_column_titles evaluation field provides named columns when included in a Treatment Plan evaluation template.
By default, this segments the treatment plan into three sections: Objective, Plan/Intervention, and Frequency.
You can set your preferred number of columns and titles using the Record names field. For example, let's say we wanted four columns with the following information: Short Term Goals|Plan|Duration|Modality.
Our treatment plan would now display the four columns with the updated labels. Note that the treatment plan item field type only allows for three textboxes so we recommend sticking with a three-column layout.
Treatment Plan Item
The treatment_plan_item evaluation field provides three text boxes to align with the default three-column treatment plan layout.
The treatment plan item contains three text boxes for completion. Additionally, the ability to add another item is included automatically, simply click the Add item button to include another row.
Additionally, for each item, you can use the Add Status button to document the patient's progress.
You can also pre-fill the Treatment Plan items if desired. This allows you to create standard Treatment Plan templates. Using the Record names field, enter the text you want to include in each box like so: Patient will receive an orientation to facility policies, rules, and standards.|Staff will provide orientation to the patient and provide necessary details on the date of admission.|24 hours
The item will be prefilled and can be edited by clicking the Edit icon. Pre-filling a treatment plan item will automatically remove the Add item button. You'll need to include another treatment_plan_item field if you want the therapist to have the ability to add additional treatment plan objectives, interventions, and frequencies.
Continue adding treatment plan items as needed.
Adding Problems and Goals
You may want to begin the form by adding problem and goal fields. There are no treatment-specific field types for these items. We recommend using one of the following fields based on your preference:
