9. Introduce and Assign Care Team to Patient

A Care Team consists of healthcare professionals who collaborate on the patient’s care plan, ensuring timely access to treatment services and support. Each patient should have an assigned Care Team, including a Primary Therapist, Case Manager, Primary Physician, Primary Nurse, and any other custom care team members your organization uses.

Facilities often admit patients based on the capacity of their providers. Assigning a Care Team allows tracking of individual provider capacity within the Dashboard and Kipu BI.

Note: This process only describes the admissions process for clients not using a CRM or integrated RCM platform.

Let’s review Kipu EMR’s best practice workflow for the assignment of the patient's care team.

  1. Access the Patient Chart: From the patient chart, click on the New Document icon.A screenshot of a computer

  2. Centralized Documentation: Search and select to add the Assignment of Care Team form to the patient chart.
  3. Open the form, use the drop-down list to assign each care team member, and click Update.
  4. Care Team Roles:
    • Primary Therapist
    • Case Manager
    • Primary Nurse
    • Primary Physician
    • UR Coordinator
    • Additional custom care team roles can be created and assigned. Click here for instructions.
      • Note: Custom care team roles will not have a Caseload Tab on the Dashboard. Learn more about the Dashboard Caseload tab.

Care Team Meetings

The patient’s Care Team usually meets weekly or bi-weekly to discuss the patient’s progress in treatment. Treatment plans and meetings can be documented in the patient’s chart or a group session. Some facilities choose to leave this documentation out of the patient’s record to keep sensitive information private. In such cases, use the Shift feature to document these meetings.
