Clinical Reports: Patients

The Patients Report provides data grouped by patient primarily using the information from the patient's facesheet. This report only displays the fields selected during report creation. 

How to Create a Patients Report

Super Admins, Records Admins, and users with Manage Reports feature can build the following report:

  1. Open the Reports tab and click New Report.mceclip0__3_.png
  2. Select Clinical for the report type.

  3. Enter your reporting parameters. mceclip0.png
    1. Report Name
    2. Template: Select Patients.
    3. Sort Direction: Select Ascending or Descending for the report order.
    4. Date Range: Select a date range for this report. Start Date/End Date is only available when a Custom date range is selected.
    5. Criteria: Use this drop-down to determine which patients are included in the report based on the date range selected. 
      • Admission Date: Pulls data from charts admitted in the specified date range.
      • Discharge Date: Pulls data from charts discharged in the specified date range.
      • Census: Pulls data from the charts that were in the census in the specified date range.
      • Pre Admission: Pulls data based on the charts that were in pre-admission status during the specified date range.
      • Updated At: Pulls data based on the charts that were updated within the specified date range. 
      • Expected Patients: Pulls data from patients who are expected in the facility for the date range based on the scheduled days on their insurance authorization.
        • Note: If MAT is enabled, then the Expected Patients field will pull data based on the scheduled on-site days for the MAT order. 
    6. Selection: Choose to include Patients with MR (admitted), Patients without MR (not admitted), or All Patients.
    7. Determine who can edit the report using the Editable by radio buttons.
      • All: The report can be edited by any user who has access to it.
      • Owner: The report can be edited by the report's creator or a Super Admin that has a direct URL to the report.
    8. Identify who can view the report using the Viewable by radio buttons.
      • All: The report is available under Public Reports for all users who have access to Reports.
      • Owner: The report is available under Private Reports for the current user, or by using the direct URL.
  4. Click Continue to select which fields to include in your report.
  5. Use the Data Fields Selection to include each field you'd like to see on the report like MR Number, Full Name, Location, Pre Admission Status, State, Payment Method, etc. The order of the fields selected here is how the fields will display on the report.   6. Click Run Report to view your results. mceclip2.png
  7. From here, you can edit or export the report for further manipulation.

Single Location

To run this report for a specific location in the master instance, select the Location from the header and click Refresh Data. mceclip3.png

Patients Data Points

Patients data points include Patients, Contacts, Insurances, Allergies, Diets, External Apps, Care Team, and External ID Mapping, and mostly pull information from corresponding facesheet areas. This area is also used in other report types such as Patients Evaluations, or Billable items reports.

While many data points return expected results, there are a few that could use some additional explanation. Please review the following list: 

  • Allow UR: This field returns a true/false result where 
    • True: Patient chart is enabled for UR Access
    • False: Patient chart is not enabled for UR Access
  • Chart ID: Patient chart's unique ID, part of URL
  • Closed: True/false on whether the patient chart has been closed.
  • Closed By: The user who closed the chart (i.e. last updated the chart)
  • eRx Preferred Pharmacy: True/false (for eRx clients only). 
    • True: Patient has a preferred pharmacy
    • False: Patient doesn't have a preferred pharmacy added to their chart.
  • First Contact Information: This is the phone number of the first contact
  • Food Restrictions: Not used, please refer to the Diet area.
  • Guardian Signature Required:
    • True: The patient has Guardian contact
    • False: The patient has no Guardian contact
  • Kis Share Document: ID of the document pushed with the patient's facesheet through the Transfer feature
  • Master ID: This is the patient chart internal ID and is not visible in the chart. Used primarily for the troubleshooting purposes.
  • One Time Only Patient: Returns a true/false result based on if the patient is marked as OTO.
  • Patient Color Date: This is the date the patient was assigned to the Program.
  • Patient Diets Count: The number of Diets. For detailed Foor Restriction information, refer to the Diets section.
  • Pharmacy Company Fax: This field is not currently used in the EMR.
  • Pharmacy Company Phone: This field is not currently used in the EMR.
  • Pre Admission Status Short Code: This field is currently not used/available in the application
  • Program: This displays the patient's program during the report date parameters. To view a history of programs for the patient please use the History Report.
  • Referrer Contact Required: This field returns a true/false result.
  • Referrer Contact: For deeper reporting including statistics on referrals, check out the Operations Referral Report.
  • Unreviewed Orders: The number of unsigned orders
  • Updated By: The user who last updated Patient Information or Payment Method

Information Pulled From Forms

The following data points are pulled from patient forms (Consents/Evaluations) and are not present on the facesheet. 

  • Current BMI
  • Current Weight
  • Height
  • Locker
  • Drug of Choice
    • Drug of Choice - Primary
    • Drug of Choice - Secondary
    • Drug of Choice - Tertiary

External Apps

The External Apps section pulls information about any external apps added to the patient's chart (such as CollaborateMD). These fields are: 

  • App Name
  • App ID
