How do I delete entries on the Height & Weight History window without red Xs?

Issue

I need to delete an entry from the Height & Weight History window, but it doesn't have the red X that allows me to delete it.

Keep in Mind
Your facility may have named the tabs referenced as Doctor's Orders and Med Log a different name.

Environment

Kipu EMR, Chart: Doctor's Orders Tab, Height and Weight Logs, Evaluations, Roles: Super Admin, Records Admin, Doctor, and Nurse

Resolution

Entries without a red X in the Height & Weight History window occur when a facility uses both the  Doctor's Orders tab and evaluation forms to enter height and weight information. Entries with the red X were entered from the Doctor's Orders tab and can be removed by clicking on the X, whereas the ones without the red X must be removed from the evaluation where they were entered.

To remove the Height & Weight from the Doctor's Orders tab:

1. Open the Doctor's Orders tab and select the History button.
   
2. Select the red X next to the entry you need to remove.
   
3. A pop-up appears letting you know that the notification was successfully removed. You can click on the Hide Deleted button to hide deleted entries.
   
4. Once you hide entries, there will be a Show Deleted entries button so that you still have access to the deleted entries, but they no longer appear in the list.
   

To remove the height and weight from a completed form, the user must either be a Super Admin or a Records Admin. If the form has not been signed and completed, any user who normally has access to it will still be able to make changes.

1. Open the Doctor's Orders tab and select the History button.
   
2. Click on the Date/Time for the entry that you want to remove.
   
3. You will be redirected to the evaluation form where this information was entered.
   
4. Locate the Height & Weight section of the form and remove all previously entered information including the date/time. 
   1. Note: Unless you have the Manage Target Body Weight role enabled on your user, the Target Body Weight field will be grayed out and you will not be able to edit it.
5. Once you have edited the information, be sure to scroll to the bottom of the form and save your changes by clicking Update.
6. The entry added by form is now removed from the Height & Weight History list.
   

Cause

Because the data is entered in different ways they must be removed from where they were entered in order to reflect within the History the same way.
