Why Do I See Forms Duplicate When Pressing ‘Populate With Forms’?

Issue

When selecting the Populate with forms button, duplicate forms appear in the patient chart tab (e.g., two Activities Releases). 

Environment

Kipu EMR, Roles: Super Admin, Features: Manage Templates, Templates: Consents & Evaluations

Resolution

This occurs for two reasons, 1) because there are two of the same form available in the Templates section for this tab, 2) the facility previously used Kipu Messenger, which required Consent forms to be duplicated as Evaluations forms to be sent to the patient's portal.

To resolve this, either a Super Admin or a user with the Manage Templates feature will need to navigate to the Templates tab to update these forms.

  1. Click on the Templates tab towards the top of your screen.
  2. Search both Consent forms and Evaluations tabs to see if there are duplicate versions of the same forms. Select the edit pencil for the form you need to disable.

  3. Uncheck the Enabled box and click Update to confirm the saved changes.
