Discharge Type

Discharge Type is a crucial criteria for reporting and analyzing treatment outcomes. Discharge Type is built into evaluation forms using the field type patient.discarge_type

Standard Discharge Types

The EMR contains a standardized hard-coded list of common discharge types tracked by accreditation organizations and state regulatory agencies.

  • Completed treatment: Usually used for treatment that includes controlled substances.
  • Completed episode of care (substance free for at least 30 Days): Used for treatment that does not include controlled substances.
  • Left voluntarily before completing treatment (against medical, clinical or staff advice): AMA discharges.
  • Left involuntarily before completing treatment: is commonly used when the patient is referred to another (usually higher) level of care, e.g., if the patient was sent to the hospital due to medical complications.
  • Referred outside of agency - episode of care completed
  • Referred outside of agency - episode of care not completed
  • Administrative discharge (initiated by the agency)
  • Non-compliant with agency's rules
  • Incarcerated
  • Deceased
  • Agency closed with no referral

Customize Discharge Types

Super Admins can add, disable, and rename Discharge Types in SettingsPatients

  1. Click on your initials and select Settings.

  2. Open the Patients tab. This tab can be named something else but will always be the first tab in your settings menu. 
  3. Scroll to the Discharge Types section.
  4. Here, you can: mceclip1.png
    1. Rename the discharge type by adding a Display Name, e.g., if you prefer to use "Left A.M.A." instead of the full type's title. This label will be used in evaluations, on the facesheet, and in reports. 
    2. Add a new discharge type by clicking Add item. Enter the title and click Update to save the changes.
    3. Disable a discharge type by unchecking the Enabled box. This prevents the discharge type from being selected. 

Discharge Field in Evaluations

Once Discharge Types are set up and ready to use, Super Admins, a user with Manage Templates or 
Manage Templates - consents and evaluations features can add patient.discharge_type field type to any form that may require it, or create a standalone form to manage discharge types.

When adding a discharge form to the patient's chart, the field appears as a drop-down containing the discharge types configured in your settings. Click here to learn more about discharging a patient.

After the documentation has been completed, the discharge type displays in the patient facesheet for quick reference.mceclip2.png

If the form is deleted but the Discharge Type is not changed back to the blank option at the top of the drop-down list it will continue to show on the facesheet, even if the patient is re-admitted and a new episode of care is created. The form must be restored by a Super Admin and updated before being deleted again.

Discharge Type Reporting

Discharge Type is a reportable field and is included as a key data point in Clinical and Operational reports. Additionally, you can also view Discharge information in the BI Dashboards. Read more about each report type in the corresponding articles:
