Issue
I need to update the signature requirements on some of my forms and I am not sure how.
Environment
Kipu EMR, Super Admins, Features: Manage Templates, User Functions
Resolution
You may require multiple signature levels:
- Patient (and Guarantor and/or Guardian if listed in the patient's facesheet)
- Staff who perform the evaluation (listed by Function)
- Supervisory staff who will review the evaluation (also listed by functions)
Before you begin: Staff signatures are assigned by user functions, which are unique to your organization. To learn about assigning functions to users, click here.
Consents
- Begin by clicking Templates > Consent forms.

Click the applicable template to view the required signatures section.

Required Signatures: Use these checkboxes to indicate which signatures are required.
- Patient: Require the Patient's signature on this form to mark it as signed.
- Guardian: Require the patient's guardian to sign this form. Click here to learn more about the guardian functionality.
- Guarantor: This checkbox is typically used for financial documentation. Click here to learn about the guarantor functionality.
- Staff: Require a staff member to sign this form. Any staff user can sign the form regardless of their assigned function.
Evaluations
- Begin by clicking Templates > Evaluations.

Click the applicable template to view the required signatures section.

Required Signatures: Use these checkboxes to indicate which signatures are required.
- Patient: Requires the patient to sign first.
- External: Requires that the guarantor and/or guardian sign next.

Staff signature user titles: This section determines what staff functions need to sign the evaluation before it gets reviewed.
- You can select multiple user titles to allow those staff members to sign the evaluation.
- The Force Signatures option will require the signature of at least one staff member per selected function before being moved to the Ready for Review status.

Staff review signature user titles: This section determines which staff members need to review and sign the evaluation before it is completed.
- You can select multiple user titles to allow those staff members to review the evaluation.
- The Force Signatures option will require a review signature of at least one staff member per selected function before being moved to the Complete status.
Review Signature Satisfied
This option allows users who are selected for both Staff and Review signatures to sign once instead of twice. Click here to learn more.
Additional Information
Please note that all changes to templates do not affect forms created from the template in the past. Changes will only apply to future forms using the template.
