How Do I Create an Attachment Form?

I need the ability to upload an attachment to a patient's chart.

Environment

Kipu EMR, Roles: Super Admin, Features: Manage Templates, Templates: Evaluations

Resolution

To create a new attachment form please follow the steps below:

  1. Select Templates then Evaluations.
  2. Select the New Evaluation button and you will be directed to a new page to start your evaluation.
  3.  Name the form, then select the Enabled checkbox, and click Add.
  4. Use the Patient Process drop-down to select the tab where the form will be available.
  5.  If you would like users with the Tech role to access this form, select Allow Tech Access.
  6.  To use the form in the Kipu Portal, click the Available checkbox under the Portal section.
  7. Select the boxes under Staff signature user titles for the users that need access to the form.
    • If you are unsure of which roles the abbreviations stand for, you can find the list under Settings > Users.
  8.  Select the Add Item option to create a new field type.

  9. Enter the name of the item, and then use the Label field to indicate what you want the user to see. For example, a question to be answered, a title, etc.

    Each name in your form should be unique, otherwise, when you use the edit pencil at the top of the template you may have a hard time finding the items you want to move around. The name is shown when the Form is validated by the user but is not otherwise displayed. The number that appears above the field is unique to that item and can be used to pre-populate the information into subsequent evaluations.

  10. Select Attachments from the Field type drop-down.
  11. Be sure to save your changes by clicking the Update button.
