Patient Chart: Overview

The Patient Chart is the individual file for the patient's episode of care and is where all documentation is recorded or added.

The chart is made up of two key sections: the chart header and the chart tabs. Because the Kipu EMR is customizable, it's likely that the data displayed and tab availability/nomenclature in your environment may be different from what is shown in these screenshots. 

Chart Header

The chart displays the following key information on the left side of the header. This header is persistent regardless of which patient tab you're on.

The right side of the header contains key actions that you can complete on the patient chart. 13.png

  1. Print: Use this icon to print the current tab that you're on in the patient chart. Some organizations may print the patient info section and have the patient review their current information for accuracy. 
  2. New Document: Click on this icon to open the Centralized Documentation modal.
  3. New Appointments: If you have Scheduler enabled, click here to view all scheduled appointments. If you are a Super Admin or have Scheduler permissions, you will be able to schedule new appointments here as well.
  4. Print Wristbands: Use this icon to print wristband tags. This icon only appears on the information tab.
  5. PDF: This icon generates a PDF of whatever tab you have open. 
  6. PDF Packages: This icon allows you to choose which information to include into a convenient PDF package.
  7. Casefile: This icon allows you to export a patient’s entire medical record as a PDF.
  8. Transfer: This icon allows you to move patient charts between different Kipu EMRs and/or Locations with Case Transfer

Other Icons:

  1. Share Portal Package: Share information to the Patient Portal.
  2. Kipu Intelligence: When Kipu Intelligence is enabled, click on this icon to access the feature.
  3. JSON Export: Super Admins can click on this icon to download the patient's facesheet as a JSON file.

Chart Tabs

Each patient chart contains multiple tabs. The names, functions, and availability of tabs are determined by the Patient Processes settings chosen by your organization. mceclip8.png

Default Tabs

There are three tabs that appear on every patient chart regardless of settings:

  • Information tab: This will always be the first tab in the list and houses the patient facesheet. 
  • Attendance tab: This tab is related to the Attendance Tracker feature. If the Show Attendance instance setting is enabled, this tab is added after your custom tabs.
  • Chart Summary: This will always be the last tab on the patient chart and provides a chronological overview of the chart information. If you’ve enabled Chart Check an additional tab will be added to this view.

Additional Tabs

Typically charts will contain multiple Forms tabs that house patient documentation (consents and evaluations). These tabs can often be segmented based on patient lifecycle (e.g., Admission, Discharge) or based on user role (e.g., Clinical, Nursing). mceclip0.png

The other tabs in the chart are called Functional tabs and relate to specific features within the Kipu EMR. 

  • Orders: Create/update Medication and Action Orders. Discontinue Orders. View/collect key integrated assessments. Use Fax to Pharmacy or eRx to send prescriptions to pharmacies.
  • MARs (Med Log/MORs): Record the administration of medications and completion of action orders. Collect key integrated assessments. blobid3.png
  • Group Sessions: When Group Session functionality is in use, all documented Group Sessions, with individual and group notes, will be recorded here. blobid0.png
  • PHI: Log any PHI request or disclosure.blobid4.png
  • Labs Requisitions: Available when a Lab Interface is enabled, this tab lists all Lab Requisitions and their status. blobid6.png
  • Lab Results: Available when a Lab Interface is enabled, this tab allows you to review the patient's results. blobid12.png
  • Lab Orders: Available when a Lab Interface is enabled, this tab allows you to create lab orders. blobid10.png
  • Patient Assessments: This tab allows you to enroll and unenroll the patient in outcomes programs, send individual assessments, and evaluate the patient's progress. This tab is available for organizations using Outcomes. blobid14.png
  • Rounds: For organizations using the Rounds feature, this tab allows users to assign rounds to patients and view rounds history. blobid16.png
  • Billing: For organizations using Patient Ledger or integrated billing, this tab allows users to collect payments (ledger) and view/submit charges to the integrated billing software from the Billing report. This tab functions in the same manner as the Billing Audit Tool tab under Reports. It also allows users to review diagnosis codes.
  • Patient Flags: View list of all patient flags (open, resolved, or canceled) assigned to the patient.blobid20.png
